For decades Medi-Cal has been grappling with how best to care for Californians in the community with the most complex medical and social needs. Recently, under CalAIM, it has identified 10 “Populations of Focus” (PoFs) where Enhanced Care Management and Community Supports could make a tremendous difference in the quality of patients’ lives.
On TUESDAY, 7/16, we’ll touch on all 10 of these Populations of Focus, with a guide into our deep dive into Populations of Focus #5:
During our Members Discussion, we’ll look to hear how you are – or anticipate – working with CalAIM and their 10 “Populations of Focus” for Enhanced Care Management …, and the impact you believe these focuses will have on everyone else in Medi-Cal:
1. Individuals and families experiencing homelessness
2. High utilizers (those with frequent emergency room visits)
3. Adults with serious mental illness (SMI) or substance use disorder (SUD)
4. Children/youth with serious emotional disturbance or at clinical high risk for psychosis
5. Adults Living in the Community and at Risk for Long-term Care Institutionalization
6. Individuals transitioning from incarceration
7. Nursing facility residents transitioning to the community
8. Individuals involved in child welfare, including those in foster care up to age 26
9. Individuals enrolled in California Children’s Services or the CCS Whole Child Model with additional needs
10. Pregnant and postpartum individuals who meet other qualifying criteria
